Health Form Needs to be completed each year and by each new scout.  Parts A and C are required.  Part B would need to be completed by physician for any long trip.
 
Activity Consent Form  To be filled out and signed by parent when bringing a guest to an activitiy.
 
Sea Scout Personal Advancement Record:   To keep track of your advancement from     Apprentice to Ordinary to Able to Quartermaster. Be sure to have the skipper sign   off on advancements as you complete them.

To order items like sea scout manuals, uniforms or patches, or when turning in money for dues or activities, please download and print out the Remittance Form and bring it with you to the next meeting.  We are often able to get the ordered items within a week or two if we don't have them in stock.
